All volunteers are required to complete the following forms, where applicable:
In addition, all volunteers will undergo a Complete Background Check that may require: driver’s license, social security number. The cost of the Complete Background Check is FREE. If you are a student, or professor, a copy of your Student or Employee ID is also required. These precautions are mandatory and have been implemented for the safety of our students.
